New Patient Forms
In preparation for your first visit with us, we ask that you print out the forms and fill them out as best as you can. If you have questions bring the forms with you and we will attend to them during your interview.
You will also need to have 1 fleets enema 2 to 3 hours prior to your scheduled appointment.
Pre-appointment enema is not needed if:
You are having anal pain, and bleeding, or if you had anorectal surgeries within 6 weeks.
You are planning to have a consultation for a colonoscopy.
Please call us if you are uncertain about whether or not you need enema, or if you have any questions or concerns.
Please bring your insurance card and co-payment to your first appointment. For co-payment, we accept cash, check and major credit cards.
If your doctor has ordered a colonoscopy, you may be able to have this scheduled through our office without an initial consultation. You will meet your physician at the time of the procedure.
If you wish to take advantage of this service, please complete the new patient forms and contact our office to schedule your procedure. We also require an order and medical records from your referring physician. You may also download the preprocedure preparation instructions. When you call our office to schedule your procedure, we will instruct you as to the exact preparation you will require
